RTC Participation Waiver

I acknowledge that I am the parent or legal guardian of registered participant, As lawful consideration for my minor child being permitted to participate in the Resolute Lacrosse League, Team, Camp, Clinic, or any other activity sponsored by or through Resolute Lacrosse/J50 Performance. I agree that neither my minor child nor I will make a claim against, sue, attach the property of or prosecute Resolute Lacrosse/J50 Performance and their agents, sponsors, building contractors, suppliers, volunteers and employees for damages for death, personal injury or property damage which my minor child may sustain as a result of my child’s participation in these sporting activities. This release is intended to discharge in advance Resolute Lacrosse/J50 Performance and their agents, sponsors, building contractors, suppliers, volunteers and employees from and against any and all liability, including for negligent actions, arising out of or connected in any way with my minor child’s participation in the sports league, team, program, camp, clinic or any other activity. I FURTHER UNDERSTAND THAT SPORTS INVOLVE PHYSICAL CONTACT BETWEEN PLAYERS, THAT SERIOUS ACCIDENTS OCCASIONALLY OCCUR DURING SUCH SPORTING ACTIVITIES, AND THAT PARTICIPANTS IN SUCH SPORTING ACTIVITIES OCCASIONALLY SUSTAIN SERIOUS PERSONAL INJURIES (INCLUDING DEATH) AND/OR PROPERTY DAMAGE, AS A CONSEQUENCE THEREOF. KNOWING THE RISKS OF PARTICIPATION, NEVERTHELESS, I HEREBY AGREE THAT MY MINOR CHILD AND I ASSUME THOSE RISKS AND RELEASE AND HOLD HARMLESS RESOLUTE LACROSSE/J50 Performance AND THEIR AGENTS, SPONSORS, VOLUNTEERS AND EMPLOYEES WHO THROUGH NEGLIGENCE OR CARELESSNESS MIGHT OTHERWISE BE LIABLE TO ME, MY MINOR CHILD OR OUR HEIRS OR ASSIGNS FOR DAMAGES. I attest that I am eighteen (18) years old or older and that my child is physically fit and have no known medical conditions, which prohibit participation in this sport. My child and I agree to follow all laws, rules and guidelines regulating the conduct of the league, team, camp or clinic. I understand and agree that my child and I are responsible for the mechanical and/or operating condition of any and all sporting equipment provided by my child or by me for my child’s use, and I agree that my child and I will continuously inspect and maintain all equipment used, even if we have obtained any of the equipment from Resolute Lacrosse/J50 Performance, their agents, sponsors, volunteers and/or employees. I HAVE CAREFULLY READ THIS AGREEMENT AND FULLY UNDERSTAND ITS CONTENTS. I AM AWARE THAT THIS IS A RELEASE OF LIABILITY FOR MYSELF AND MY CHILD AND A CONTRACT BETWEEN MYSELF, MY CHILD AND RESOLUTE LACROSSE/J50 Perfromance AND THEIR AGENTS, SPONSORS, VOLUNTEERS AND EMPLOYEES, AND I HAVE SIGNED IT OF MY OWN FREE WILL. I also agree that Resolute Lacrosse/J50 Performance and their agents, sponsors, volunteers, and employees may use my child’s photograph in future promotions. ”All Registration Fees associated with this program are non-refundable. By signing up for registration and the payments required, you will be responsible for all registration fees in the event you are not able to complete participation in the season.”

Refund Policy

All payments made for the Resolute Elite Teams and Resolute events / tournaments are non-refundable.

However, Sports Fee Insurance will refund you, and pay what you still owe, In the event of

Injuries – Injuries that leave the athlete unable to play for 30+ days

Illness – Illness that renders the athlete unable to play for 30+ days

Death – The Death of the Athlete or Parent

Mental Illness – Player forced to withdraw from the program due to a Mental Illness

Job Relocation – Moving out of the area for your employer greater than 75 miles

Job Loss – A loss of employment in the household from a W2 employee

Get Covered Now: You need to purchase Sports fee Insurance to be covered from the unexpected. Sports Fee Insurance is a one-time payment and will cover your athlete, you, and your wallet for the entire season.

Weather Policy:
Safety is our priority for events, leagues and practices and if weather puts the safety of participating athletes at risk we will delay or cancel the event. If an event is canceled due to weather we will do our best to accommodate (makeup sessions, alter schedules, etc.) when possible. However, if we are not able to accommodate delays or cancellations, no refund will be issued.

Gift Card Policy

GIFT VOUCHERS ARE VALID FOR 24 MONTHS FROM DATE OF ISSUE. UNUSED VALUES WILL NOT BE REFUNDED OR ATTRIBUTED TO ANY TRANSACTIONS AFTER THE EXPIRY DATE.

GIFT VOUCHERS CAN BE REDEEMED AGAINST ALL PRODUCTS ONLY AT RTC.

GIFT VOUCHERS CANNOT BE REDEEMED FOR CASH, RETURNED FOR A REFUND, HAVE THEIR BALANCES CONSOLIDATED TO A NEW VOUCHER OR BE REPLACED AFTER EXPIRY AND ARE NOT LEGAL TENDER, ACCOUNT CARDS, CREDIT OR DEBIT CARD
